Can a provider number be deactivated without notification?
• Warrants or documents mailed to the service, business or pay-to address were returned by the United States Postal Service as undeliverable. • A claim has not been submitted for reimbursement from the Medi-Cal program for one year. Prior to deactivating a provider number, DHCS makes an attempt to contact the provider by telephone or in writing. If unable to make contact, DHCS is required to deactivate the provider number immediately without further notice.
