Programs that wish to submit an Enrollment form and an Application at the same time should adhere to the following procedures: 1. Send the Enrollment form and Application together in one envelope and mail it to the P.O. Box indicated on the Application form. 2. Make sure to include a note with your Application stating that you are submitting your Enrollment and Application forms together.
