The response you receive is yours to do with as you wish. State licensing boards and insurance organizations have been known to require a copy of a practitioners self-query response as part of their application process. Reportedly, some have requested the response in the original sealed envelope. If you wish, you may forward the unopened response to the requesting organization. The Data Bank will not mail the response to a third party. The Data Bank does not regulate such arrangements. You may also request and pay for additional copies of the search results when you complete the search for reports (self-query) form. Please note: a copy of a practitioner’s self-query response does not satisfy a hospital’s legal requirement to query the National Practitioner Data Bank.
