It is not possible to combine areas (multi-county, city-county combinations, regional collaborative, regional nonprofit organizations, etc.) to reach the required 500,000 population size. Large counties are encourged to apply on their own to have the best chance to be funded. However, there is no prohibition against coordination among the applicants within your state, so that your Community Transformation Grant implementation plan could be mutually supporting across the areas within your state to present a cohesive package. However, separate applications would need to be submitted. As identified in the FOA, applicants applying to serve a large county must include letters of support from ALL health departments (e.g., city and county health departments, if they exist) located within and serving all or parts of t
