Can a claim be appealed if more than 120 days have passed since the finalized date?
Yes. Submit a written appeal and INCLUDE THE REASON FOR FILING THE LATE APPEAL. Your appeal request should contain the following information: • Beneficiary name • Medicare health insurance claim (HIC) number • The specific service(s) and/or item(s) for which the redetermination is being requested • The specific date(s) of the service • The name and signature of the requestor You can find additional information located on the Highmark Medicare Services website Appeals Center.
