If you purchase a frame at a Private Practice Network Provider that is not included in the Frame Selection, but is $50 or less at wholesale, the frame is covered-in-full after the $25 co-payment. If you choose a frame with a wholesale cost greater than $50, you only pay the difference between the wholesale cost of the frame and the $50 allowance, plus the $25 co-payment. For example, if you purchase a frame that is not included on ‘s “covered-in-full” Frame Selection, what you pay will depend on the wholesale cost. If the wholesale cost of the frame is $50 or less, you are only responsible for the $25 co-payment. If the wholesale cost of the frame is $60, you would pay the difference of $10 for the frames, plus the $25 co-payment for a total payment of $35.
