As a Physical Therapist, I am working as a PRN (per diem) at a local hospital; how do I obtain coverage?
As a PRN, you should apply for coverage as a self-employed individual. You can click here for a quick quote and our on-line application form. Upon receipt of your application, Healthcare Providers Service Organization will process the application and, if approved, a certificate of insurance and bill will be sent to you. If immediate coverage is required, please provide a fax number and indicate on the application that you would prefer a faxed certificate as soon as possible.
