Are transition care recipients allowed to take leave from their transition care episode?

No. There are no leave provisions for transition care. Clients wanting to take leave must be discharged from the program. For them to re-enter the Transition Care Program, they would need to be re-admitted to hospital and meet the transition care eligibility criteria such as having completed any necessary acute and/or subacute care to be re-assessed and re-approved for a new transition care episode. However, it is possible for a transition care recipient to move from one care setting to another within the same transition care episode, e.g. from the residential to the community setting (see Question 12). These care recipients will be provided with transition care throughout that episode and do not require another ACAT assessment to enable that move.
