The best example is Missionary Ridge, the ultimate soldier’s battle. It is a mixed bag in that it is not strictly speaking simply explained by frenzied soldiers. There was also a disconnect between high command and line officers, many of whom really didn’t understand the attack on the rebel lines was supposed to stop at the advanced trench line below the ridge. Regardless, from the viewpoint of the senior commanders, (some) divisional, Corps and higher, the attack went forward without orders.
