Are there any services that require prior notification to OptiCare?
The only service that Opticare requires prior notification for is medically necessary contact lenses for conditions or reasons other than Aphakia, Keratoconus, and Bandage. You may download a Special Request form and fax it to the Medical Management Department, 252-451-2908. As a reminder, the Optometrist or Ophthalmologist must certify that the contact lenses are indicated due to a medical reason.
