Yes – recorded in the incident reports. The issues are generally down to free flow and/or the diver inhaling water following regulator switching due to either poor technique or caused by a faulty regulator. This situation reflects that encountered by a diver swapping to a stowed regulator. However, where both divers are required to switch regulators, there are two opportunities for the switch to fail.
