According to the HAO Mentor’s Manual, one’s local Community Care Access Center (CCAC) should be approached. The Toronto Central CCAC’s head office is located downtown Toronto, and their website is here. Once a hospice is affiliated with its CCAC, it benefits from referrals, information about community services, letters of support, and funding. Another important external relationship is with the local District Health Council (DHC), which exists to strengthen its community’s health care system. A third party seems to have acquired the rights to Toronto DHC’s domain name, but they can be contacted at 416-222-6522. The HAO Mentor’s Manual suggests that a hospice should be aware of their DHC’s annual plan, and likewise ensure that the DHC knows their strategic plan.
