Are there any chart systems that use voice recognition yet?
Yes, the latest version of voice recognition software is being used in electronic medical records and is quite good. Most systems use a combination of templates and voice commands to reduce the amount of repetitive work. Most voice recognition systems will perform well with continuous speech at 95-98% accuracy when trained appropriately. Do not expect to be able to dictate in the same manner as you would using a Dictaphone. In fact, this is probably not desirable as there is much redundancy in this type of record keeping.
