Are there additional survey instructions?
To help you complete the survey, there are both a worksheet with blanks coinciding with each entry and an additional instructions sheet. The additional instructions provide added information to help you know what exactly to put in each blank. Furthermore, while taking the survey, there will be question mark icons next to some items. Selecting this link will open up another browser window or tab that will provide additional information for the given question. This information is identical to what is found in the additional instructions sheet. If a difference is found, please let us know, but the printable worksheet is the official version. The worksheet and the instructions sheet are both linked from the first page of the survey.
