They will not do anything unless you ask them to. Things that you can do include paying for cabin decorations to be put up. It cost about $30 for them to decorate your cabin with streamers and a door poster. You can also ask your waiter and/or matre D to bring a cake for the birthday date and they will bring it and the waiters will sing. Royal Caribbean also has a couple other things you can order for birthday celebrations listed on their web site.
