Are routine staging labs valid in the myeloma patient on dialysis?
Routine screening labs are appropriate for myeloma patients on dialysis, except probably the creatinine, bun, and the 24-hour urine labs. The serum free light chain assay has been studied in patients with renal failure and shown to accurately detect and identify myeloma patients if the reference range is extended from the normal 0.26 – 1.65 to 0.37 – 3.1 (Hutchinson CA et al. BMC Nephrol. 2008;9:11). All other screening tests are appropriate.
