Pharmaceutical Grade means the product must be in excess of 99% purity without binders, fillers, dyes, or unknown substances. I think they are worth the money only if they also meet the United States Pharmacopeia standards, for dissolution so you can reap the benefits of what you paid for. Try it. Primarily it is a matter of concentration. If you can take smaller doses of the “pharmaceutical grade”, then it is worth it! the pharm grade has a consistant and reliable source of the active ingrediants or in this case, the fish oil in this case. the over the counter can have contaminants, and even variations in the amount of omega 3’s in pill to pill or batch to batch. Pharm grade it will be consistant. For supplements, pharm grade is better, but costs more due to the amount of work that goes into it compared to the mom and pop stuff.
