they are similar in principal but smaller, some are air cooled, whereas most cars engines are not, and can be either diesel or petrol, commercial vehicles have large diesel engines in them. car mechanics do not like working on little engines as they are too fiddle for them, like lawn mowers or bikes, and they hate large diesel engines, especially say a really dirty farm tractor engine, i think you’ll find, this holds good for the m/cycle and commercial fitters, and none like diesel train engines, due to the size, so while they will know how they work, far better than other people, they’ll lack the hands on experience, for identifying specific faults and problems, its a case of each to it’s own.
