Are medical history records required as a part of the enrollment process?
Petplan does require your pet’s medical records after enrollment. You will need to complete and sign your Medical History Record Release Form. Please give this form to every veterinarian who has examined your pet. Any medical conditions that have been noted on the records that are not fully resolved or which are likely to continue, re-occur or lead to a related illness or injury in the future will be excluded from your policy coverage.
