Are healthcare providers required to ensure that parents complete and sign the Disclosure/Destruction Directive Form?
No. Healthcare providers are ONLY required to provide the information by giving the form to the parent, legal guardian, or managing conservator. The provider would then indicate on the demographic portion of the collection kit that the form has been provided. After reviewing the Disclosure/Destruction Directive Form, the parent, legal guardian, or managing conservator should ONLY complete the form and send it to DSHS if they wish to have their childs specimen(s) destroyed.
