Please call if your health insurance is not listed above. Because each of these insurance companies provide many different insurance policies, it is your responsibility to determine if your particular policy covers psychological services, requires referral and/or pre-approval for services. Can I still see Dr. Look if my health insurance is not listed above? Yes. Dr. Look will provide a statement of services to you that you can use to file a claim with your insurance company.
