Are career counseling and occupational therapy considered the same programs?

Not at all, OT is all about helping people get back to their “occupations” (ie, jobs, housework, life in general) after an accident or event has led to a reduction in their mobility and abilities. It’s about finding new ways of doing things, new tools to help you do things etc. OT relates to your whole life, not just your career and is all about helping you get back to your life before the accident or event.
