Are agencies to submit their own model of case management services?
Through this RFP, agencies are applying to participate in a coordinated case management system governed by the policies and procedures of the Northeastern Illinois HIV/AIDS Case Management Cooperative. Agencies may adapt the Ryan White and DRS models to effectively serve their target population. However, agencies must justify any variation from the model of case management.
