This contact is reported on the unit’s Form I (Written Contacts Tally & Summary). The written contact should only be counted once by the individual who receives/intakes the document. The person translating the birth verification document will tally the contact in the appropriate language column on the Tally Sheet. The other employee who completes the English transaction will tally in the “English” column on the Tally Sheet.
