As of October 4th, 2004, vendors must submit their Transaction Fee Reporting Form (PUR 3776) through the online Transaction Fee Reporting System available from the vendor?s MyFloridaMarketPlace vendor registration account located at https://vendor.myfloridamarketplace.com. Under this streamlined reporting process, vendors are not required to submit paper copy of Transaction Fee Reporting Form (PUR 3776) with their payment. However, Vendors should reference the Consolidated Billing Invoice (CBI) number on their check to ensure proper application of payment against Transaction Fees owed.
