The employee must complete an Employee Enrollment/Change (722.9 KB) form (HCA 50-400) and a Spouse or Qualified Domestic Partner Certification (706.2 KB) form (HCA 50-704), within 60 days from the date of marriage. Coverage is effective the first of the month following the date of marriage unless the date of marriage is the first calendar day of the month. In that case, coverage would be effective on the date of marriage.
