The medical assistant is not the same person who delivered the direct client service. How is this activity coded? Close call because the question would be is this related to a direct service (Code E) or is it a referral because it’s done by a different person (B1)? The examples in the guide are somewhat inconsistent because it’s unclear as to what the definition of “follow-up” is for direct services. Best advice we can offer in looking at scenarios such as this one is to first look at the activity that is being performed, and then who is performing it. In this case the activity that is being performed is the follow-up contact with a client, it is not the work directly associated with performing the lab test.
