Reconsideration can only be performed if the hearing screen, along with a written request for reconsideration and a copy of the partial approval/denial letter, is received within 35 calendar days from the date of the letter. If the hearing evaluation is not submitted within the reconsideration time frame, a new packet will have to be submitted, with the hearing screen included. We received a 12-month authorization for a child. Just two months into the authorization, the family moved away, and the child was discharged. Three months later, the family moved back and the parent/guardian wants the child to come back to the center. Do I have to send in a new prior authorization request? No. Just send in a request asking to reinstate the prior authorization. Be sure to include the child’s Medicaid number, the prior authorization number and the day that the child started back.
