A mystery indeed. The very simple answer is that we don’t know the character that killed Kaji (we NEVER see them in the series or movies). It was most likely a member of NERV, SEELE or the JSSDF. This mystery is never resolved (and probably never will be). GAINAX officially announced that it was a “third person” from SEELE or one of the intelligence division from NERV.
