Form E is the payment form that accompanies your check payable and mailed to the Diocesan Office of Youth Ministry. Form E Please go to the Download forms section on this site and complete the Diocese of Fort Worth Payment Form E, and make your check payable to The Diocese of Fort Worth. Registration Fee if all forms mailed by April 1, 2009 – $40 per person Registration Fee if all forms mailed after April 1, 2009 – $50 per person Make 2 copies of Form E, and one photo copy of your check. What is Form F? Form F Please go to the Download forms section on this site and complete the Diocesan Electronic Registration Form (Form F) and email to jcastillo@fwdioc.org and to y2kregistration@hotmail.com * This form must be submitted electronically.
