Lines 1-4 on the MICS work fine, but I can get the lines on the 2nd LS/DS trunk cartridge to work. Why?

You need to use lines 25-28 in programming for the 2nd LS/DS card slot in the Main KSU. Lines 1-24 are dedicated to the 1st card slot in case T-1 is used, therefore the starting line for programming the card slot is line 25.
