What is the Maryland P3 Program?
• The Maryland P3 Program is designed to provide patient education and self-management training to patients who have diabetes using the American Pharmacists Association Patient-Self Management Program based on the highly successful Asheville Model. • The mission of the Maryland P3 Program is to improve the health and quality of life of those with chronic diseases while controlling overall health costs. Through the Maryland P3 Program, employers and payers encourage patient self-management with the aid of trained pharmacist coaches, physicians and other community health resources.
