How is an SAI claim filed?
It is the Safety Director’s responsibility to make sure the claimant receives an SAI Claim Form. Be sure to provide instructions in both English and Spanish. Inform the claimant on how to download the form from the AYSO Web site, www.soccer.org. The form must be completed by the adult or parent and must be signed by the Regional Commissioner and the Safety Director. It is the responsibility of the claimant to make a copy for his/her records and mail the claim form with all the relevant documents to the address provided on the form. Claims must be submitted within 90 days of the date of injury EVEN if the primary insurance has not finished paying. As the Safety Director, DO NOT promise that AYSO’s policy will cover ALL out-of-pocket expenses.
