PHA is short for Physical Health Assessment. This medical review is governed by the OPNAV Instruction 6120.3 dated 5 December 2001. PHAs are required of all active duty service members on an annual basis. It is recommended that commands complete PHAs during individual’s birth month. The PHA consists of an individual review of medical status to include items such as, blood pressure measurement, cancer and blood screening. Participants in command Physical Fitness Assessments (PFA) must complete a PARFQ and provide it to the medical representative at the time of the PHA. The medical provider will review the PARFQ and either clear the individual for both PFAs for the year or provide specific PFA medical waiver recommendations. In accordance with the OPNAVINST 6110.1G, all members are required to complete a Preventive Health Assessment (PHA) once a year generally during their birth month.
