How is FAA first class medical?

If a waiver is required due to a medical deficiency, your AME can complete the required paperwork and submit the request of the FAA in OKC. Most waivers for vision or hearing are usually granted. A first class medical certificate is valid for six calendar months. If a medical condition arises within the six months that would prohibit a pilot from obtaining a new medical certificate, then the current certificate immediately becomes invalid until the condition is treated/cured. (eg., a pilot receives a first class medical then two months later suffers a heart attack…the medical certificate immediately becomes invalid).
