The DMH Prime Time Geriatric Assessment Center is an innovative outpatient assessment program designed to assist individuals 65 and older (and their families) by identifying health needs and providing assessment, planning and intervention. An assessment includes a thorough interview with the patient and his or her family (by a Gerontological Certified Registered Nurse), a physical examination performed by a Geriatrician (a board certified physician with additional training and expertise in Geriatrics). Additional evaluations and assessments may be completed by other healthcare professionals, a physiatrist, registered dietitians, pharmacists and case managers. After the assessment is complete the interdisciplinary team members meet with the patient and family to discuss a treatment plan. What problems should be evaluated?
