Who can be admitted to Leeway?
Anyone 16 years of age or older, who has HIV/AIDS, and who needs 24-hour skilled nursing services and/or supervision is eligible for admission. In addition to nursing services, Leeway also offers IV therapy, wound care, physical therapy, occupational therapy and speech therapy, as well as supervision with medications and treatments. How does one apply to Leeway? Referrals are accepted from hospitals, home care agencies, out-patient clinics, as well as from private physicians. An application is completed and sent to the Leeway Admissions Coordinator, 40 Albert Street, New Haven, CT 06511. All necessary medical information and required tests must be completed in order to be placed on the facility waiting list. Placement on the waiting list does not guarantee admission to Leeway; it indicates only that the potential resident will be considered for admission.
