What are the requirements for Cert Name?
It must be entered as Last Name, First Name. It must match the teacher’s certificate. If the certificate has middle name/middle initial on it, then you should include that in the Cert Name field. If the staff member is not a certified staff member, then it should be Last Name, First Name. If there is more than one staff member with the same name, you can include a middle initial for the staff member so long as they are not a certified staff or if they are, the middle initial is on the certificate.
