Who does “Patient Name” refer to?
For health service organizations, the financially responsible individual(s) may not be the same as the individual on which the medical services were performed (the patient). For example, for medical services rendered for a child, the parents would be listed as the financially responsible party (“Debtor’s Name”), while the child’s name would be listed under “Patient Name”.
