How can individuals apply for PAC?
The PAC application is a relatively short mail-in application. To get an application, individuals can call 1-800-226-2142 to request an application by mail or they can download it from the web at www.dhmh.state.md.us/mma/pac. Once the application is completed it must be mailed to a central DHMH address – PAC Eligibility Services, P.O. Box 386, Baltimore, MD 21203-0386. Proof of income and citizenship must be included with the application.
