Where do I send my DD Form 1840/1840R (pink forms) for my 70 day carrier notification of damages?
Carrier: Under FRV shipments, you send your DD Form 1840/1840R to the carrier directly. During delivery, the carrier will provide you with their address. In this case, you have 75 days from delivery to file it with the carrier. Note that if you want the base legal office to forward this form to the carrier, you must bring it to the base legal office within 70 days of the delivery of your household goods. Claims Service Center: For claims adjudicated by us, you must take your DD Form 1840/1840R to the nearest legal office for dispatch within 70 days from the date of your delivery. Your DD Form 1840/1840R will also be uploaded and sent to the AF Claims Service Center. Even if you had Full Replacement Protection or you were eligible for Full Replacement Value coverage on your move, you must still submit your pink form with any additional damage within 70 days from the date of delivery.
