An eForm is an electronic evaluation program utilized by applicants and evaluators to request track and submit online Institution and Professional Evaluations. The applicant must have the email address of the Professional Evaluator(s) in order to submit an eForm request. ADEA PASS strongly encourages the use of eForms, as they expedite application processing and reduce the change of lost forms.
