A. The PAC facilitator and community liaison coordinator are your first connections to the PAC. Included in the team are the physician, psychiatrist, pharmacologist, psychologist, nurses, mental health support workers, and an occupational therapist who develop individual assessment and treatment plans in partnership with the community care teams. Other community specialists may become involved as necessary (such as dietician, podiatrist, and lab technician). Staff are experienced professionals who provide clinical care and supervision of individual’s needs on a 24-hour basis.
