How can domestic partners register in the District?
Go to the Vital Records Division at the DC Department of Health, which is in charge of maintaining the District’s domestic partners registry. You can obtain information and download forms from their website, which is athttp://dchealth.dc.gov/services/administration_offices/schs/vital_index.shtm.Signatures of each partner must be notarized on the application form. Both partners must bring the application to the Vital Records Division and provide proof of residency and identification. The fee is $45.
