What information is needed on the Reply Form?
Read the Reply Form carefully. On it you need to include your name (exactly as printed on your passport), date and place of birth, your acceptance or non-acceptance of the appointment, your signature, the days date, your current and permanent contact information, whether a spouse/partner will accompany you, the airport and city from which you will depart, your passport number and expiration date, and two passport-sized photographs taken within the last three months. DO NOT FORGET TO SIGN THE REPLY FORM.
